Agreement
I hereby agree to indemnify and hold harmless Santa Fe Christian School, it’s officers and employees, Player’s Edge Football Camp, any individual participating, organizing or co-sponsoring this football camp, for and against any and all liability for any injury which I may suffer arising out of or in any way connected with my participation in this football camp. In case of emergency, arising during or in connection with any activity of this athletic program, I authorize any person in charge of the activity to consent to medical and/or dental treatment for myself, at my expense. I understand that Santa Fe Christian School is not obligated to carry any insurance to cover those medical and/or dental expenses and that, if such insurance is carried, coverage will be provided only for expenses in excess of the limits of any other insurance providing benefits. Any disputes arising between Santa Fe Christian School and participants in this football camp will be settled by independent arbitration. I hereby declare and fully understand and agree that participating in the game of football is dangerous and can result in serious injury, permanent paralysis and even death. I declare myself physically fit and have passed the physical exam required by the football organization I will be playing for this season.
